CLIENT PAPERWORK
Please fill-out the intake form before your first session.
You may either e-mail it back to me before your appointment or bring it in, and we will go over the information during your initial session.
RESCHEDULE AN APPOINTMENT
You are able to reschedule and cancel upcoming appointments from the “Change/ Cancel Appointments” link in your appointment confirmation email.
If you have deleted your appointment confirmation email, please contact me directly for further assistance.
CANCELLATION POLICY
Please contact me at least 24 hours prior to your scheduled appointment time to reschedule or cancel your appointment to avoid being charged for the session. If you do not cancel 24 hours prior to your scheduled appointment, or you do not show up to your appointmet, you will be charged the full-session fee.
CONFIDENTIALITY
All sessions are legally confidential and the information gathered therein cannot be released without the client’s consent. There are several exceptions to confidentiality mandated by law, which we will cover during session.
